Jay, Ed, I've done a quick-concept document in which I created two columnar body flows on the master page. Anyone is welcome to a MIF copy of this empty test document. ON THE MASTER PAGE Created a narrow flow near the binding margin, labelled it Line Numbers (truth in tags!), and set it to Autoconnect. I created a new paragraph tag in it [Line Numbers] of the simplest possible autonumber format: <n+>, which is set to "end of paragraph," allowing for right-aligned numbers without leading zeroes (alternately, set paragraph to right-align). In real use, of course, one must set the paragraph spacing and font to match code samples (set a character tag and apply to both) so that the lines in each column will match up. I didn't bother creating the Left master page but it's no trick to do so, just copy and paste the text blocks from the Right page and reverse their positions on the Left page to put numbers near binding as requested. ON THE BODY PAGES Type whatever or text goes in the body flow and then create a pagefull of returns in the line-number flow and copy and paste until all your doc is numbered. (This part can undoubtedly be automated further but I only took a coffee break to create the test doc.) Although FrameMaker now lets us auto-create multi-column layouts of an even width, one still needs to understand how to create frames as columns and link them (or not) in order to create uneven-width columns. This solution applies a two-flow version of uneven columns.
