Thanks to all those who offered suggestions. The solution (workaround?) I
found to work was to copy the contents of the main file to the end of the
Title/TOC file, eliminate the book file, and regenerate the TOC within the
new file (making sure to have the cursor in the TOC). Simple, but not
recommended for large books.
