If you took the TOC and IX files you generated for each chapter and dragged them into your book file, then Frame does not recognize them as generatable files, but rather as "regular" files just like your chapters. Verify that you have created the TOC and IX by selecting Add > TOC and Add > Standard Index from the book file menu. This puts placeholders into the book file that will become files when you generate the book. One thing I learned a long time ago that has helped me get the formatting I wanted from the first time I generated the files is to *not* generate the book right away when I add these placeholders. Instead, I create a skeleton file for each generated file, with the exact name that Frame added to the book file, and put these files into the directory where the book file lives.
