You're right, the 12/99 disk has 4.06 underneath the 4.0. 

Last Q, Do I first need to Uninstall 4.0 before Installing 4.06 ?

One of the CDs you have is probably the 4.06. On my 4.06 CD, Windows
version, in very small print under 4.0 on the CD is 4.0.6, which installs as
4.05a. Update 1 to 4.05a is available on Adobe's web site. You MUST have
4.05 (4.06) installed to install the update. If you go to Adobe's site,
click on the title of the update and you will be rewarded with the readme.
It isn't packaged with the update. Dov just posted a notice of it yesterday.
