Need help with an index style

I picked up a book the other day and was looking at the index. Each level1IX began with a GroupTitle drop cap.
Like so:

Ant    where "A" is a drop cap 
antler
etc

Bravo  
etc

I need a little direction here. Can this be done with "stock" frame tools?

I have opened the Reference page to the index and inserted a Run into Paragraph anchor, but..... I need help.
