Instead of trying to figure out what you've done or what has changed, here are some general trouble-shooting tips that always apply: Are you 100% sure you have the numbering in the book file set up correctly? Check and make sure it's set to Continue for all files. Have you remembered to update the book after making any changes? Check the autonumbering for each chapter tag in each file: is it really "C:<n+>" for all of them, without any overrides? Do you use the "C:" autonumbering series for any other paragraphs? If so, make sure they don't reset the series (check overrides!). You may have an empty Chapter tag placed somewhere by mistake, or any other paragraph resetting the numbering by mistake. Try inserting a Chapter tag with C:<n+> at the end of the chapters; does it increment like it should? If it doesn't show the correct number, you probably have an override somewhere.
