those bookmarks [urgent]

All--

I need some help.  Whenever I print a book as a PDF file, my bookmarks don't
appear the way I want them to appear.

I want to do them to appear something like this:

Chapter1
|_A
|_B
|__1
|__2
|__3
|_C
Chapter2
|_A
|_B

but instead they appear all over the place.  Is there some kind of tutorial
or plug-in that will make the "perfect" book-marks layout?
