We have a similar thing with phantoms in our ToCs and indexes, in PDF only: it's as if the last link on bottom right of page 1 of the index got "carbon-copied" on to page 3 in the same position, and the same for ToC, LoF and LoT. In every case the link is hot, and goes to the identical point as the "top copy" link. We've studied our templates in great detail, but no luck. This phenomenon is definitely not present in the .fm file. WinNT, Frame 5.5.6p145, Distiller 4.05 1026 - but it was the same with Distiller 3.02.
