Problem importing XML

I seem to be having a problem with rendering imported XML.

If a paragraph starts with an inline that contains another inline the 
formatting seem to go wrong.

For exmaple, if I have XML of the form

<para><option>ttt=<replaceable>var</replaceable></option> xxxx ...


The replaceable will pick up the formatting correctly, but the text just 
in the <option> does not.

Worse the paragraph, or other container does not seem to be processed 
correctly.

Has anyone else seen anthing like this.
