I have a Win-only plugin that will "remember" page breaks
and hard line breaks in generated files. This might not be
the ideal solution, but it has saved me quite a bit of time,
especially in indexes that have thousands of entries, and
need page and line breaks.

It was available on the FrameUsers site, but I have failed
to get it up there since the big change. Try getting it
from my page...

I use JavaScript, so you have to accept scripts in your
browser security settings. There is documentation for
TocBreaker, which is the plugin that addresses this issue.
