That is incorrect. You can create a TOC reference page from scratch.
1. Choose Special > Add Reference page, and name it TOC.
2. Use the drawing tool to create a text frame in the new reference page.
3. Select the new text frame, and choose Object Properties. In the
Customize Text Frame dialog, enter TOC for the flow tag.
You can also use the procedure above to create the reference page for any
other kind of generated list or index, provided the reference page and text 
flow
names match the names conform to the names that FM requires for them.

After creating a reference page as described above, you must create the
specifications using the paragraph tagnames and building blocks required
for each type of generated list or index. Normally, you would do it this way
only if the specifications already exist in a separate file, in which case you
simply copy the entire specification set from the source file and paste them
into the newly created reference page in the target file.
