Answering my own question: With a quick look at the Adobe web site at, I found: http://www.adobe.com/supportservice/custsupport/SOLUTIONS/10e3a.htm This details using the Reference pages with the Table called BookHTML and changing the appropriate entry in the column titled "New Web Page?" from "N" to "Y". Duh!!!
