Interesting idea, but I think far too complicated for the one hotspot she asked for. Even if she planned on five or so, individual text frames are a lot more efficient because you can copy, paste, and resize them very quickly. If you create a single text frame and add a hypertext link, when you copy the frame, you also copy the hypertext link, and it's usually faster to edit the copied links and just change the destinamtion name than it is to create a new link each time. You can also put the callout in the text frame if you want.
