Thanks to all who responded to my original post earlier today (same subject). I had to re-build the book from scratch to get it to work. Many people suggested: "Import the formats from a 'good' copy of the ToC, Index, etc." Evidently, I didn't explain the problem very well. I *did* do this. For some reason, every time I re-generated (sounds like I am a Borg...), the regeneration process stripped all the formatting and structure out... creating a new ToC and Index. Too many variables to figure out what went wrong for sure. Possibly a bug. A couple of people suggested a problem with case-sensitivity in the book and generated file names... and also, having to work with Windows 3.1 file naming conventions throws another angle into it. Anyway, it's fixed now. Thanks... back to my regeneration alcove...resistance is futile...
