You can set siblings, for example, when you want both MyAppendixTitle and
MyChapterTitle to be at level one.

1     In the Enhance window, choose Outline > Select Outline Formats.

2     Click the paragraph format you want to sibble in the Body Formats
list.

3     Click the Set Secondary Level button.

4     Click the outline level radio button.

When you are demoting or promoting headings, the primary format is applied,
so if you intended a
sibling you have to apply it separately.
