It was the first of these. The target markers were in place, but the marker
content didn't match the paragraph text. (No conditions anywhere.)

Yebbut -- how come FM didn't complain during a book update, and no part of
the PDF generation process complained, and there were apparent links in the
PDF but they just didn't work?
