Better yet, try opening the TOC file in FrameMaker and then distilling one 
page at a time.  If Distiller chokes on page 2 of the TOC the error will be 
on page 2 or (who knows why?, but it happens) on the previous page (page 1).

Most Distiller/printing problems seem to involve the text/fonts you can't 
see because they're on the Master pages, so look on the Master pages of the 
TOC file for empty paragraphs that use bogus fonts, or (apparently) empty 
text frames, or graphics with embedded fonts (especially graphics assembled 
from ClipArt via MS Word or CorelDraw).  How do you find these?  Use the 
Object selection tool on the toolbar and click and/drag all over the Master 
pages...use Ctrl-click to look for objects stacked behind other objects...
