When referencing other paragraphs in a cross-ref format, the other paragraph must be in the same flow. This is probably why it doesn't work for you. Unless you can get rid of that disconnected page (why do you have it?), one option would be to make an invisible dummy paragraph on the disconnected page that cross-references the contents of the real Chapter paragraph, using a simple <$paranum> format. Then, in the troublesome cross-ref, use <$paratext[dummypara]> instead of <$paranum[Chapter]>. There are other variants of dummy paragraphs that may work for you, depending on how your documents look.
