The first idea (copied below) has worked well for me for several books. When the TOC file for the book is regenerated, the cross-references are as well. In the few instances that I've revised or deleted a heading, I've usually remembered to go back to the sub-TOC within the chapter to replace or delete the corresponding cross-reference.
