If you're sure that your TOC reference page doesn't contain an
extra tab at the beginning of some *TOC paragraphs, then it's
the headings in the chapter that must contain them.

In other words, check the very beginning of the troublesome
headings in the chapter files, with Text Symbols turned on.
Delete any spurious tabs there, but be careful not to delete
any markers.

Remember that tabs can be inserted without having any visual
effect on the text (FM isn't Word :-)
Your headings probably don't have any tab stops set, but your
entries on the TOC reference have. That's why the tabs give
a visual effect there, but not in the headings.
