If you're looking for a robust, automated solution, Miramo, by Datazone, 
can do this with ease, complete with every bell and whistle that FM can do, 
including real cross-refs, hyperlinks, multiple chapters, books, etc. In 
addition, Miramo brings lots of other capability like automatic PDF output, 
data-driven formatting and overrides to styles, etc etc.
